New Clients
Welcome to our practice! We look forward to serving you and your pet(s) and making your needs our highest priority. See below for important information about our practice, including making appointments and payments, and fill out our new client form to get started!
Pet Portal
After your first appointment with us, you’ll be given information to log in to your Pet Portal with us. Via the Pet Portal, you can access your pet’s medical records and vaccine information whenever you need it.
Appointment Policy
We require all routine or non-emergency visits to have an appointment, so we may respect the valuable time of all our clients. We will see urgent and emergency matters as needed. If you cannot make your appointment, please notify Addison County Mobile Veterinary Services immediately. We reserve the right to charge for appointments that are canceled or broken without 24 hours' notice.
Payment and Insurance Information
Payment is due at the time of service. We can provide you with a cost estimate upon request, but cannot give an estimate over the phone for non-routine procedures without seeing your pet first. We do offer financing for advanced or routine procedures through CareCredit and/or Scratchpay, however we do not offer in-house payment plans at this time. For surgical and/or sedated procedures, we may request a deposit at drop-off time.
If you have pet insurance, please bring any paperwork to your appointment that our staff may need to help you fill out. Also, bring any previous records or provide us with your previous vet’s information so we can keep your pet’s records accurate.
